Synergy Health Plans provides informational support and guidance to individuals transitioning coverage from LifeX to Synergy Health, including assistance with migration questions, account updates, payment-related steps, and general coverage transition support. The Site does not provide insurance quotes, bind coverage, or guarantee plan availability, benefits, or pricing. Plan availability, benefits, pricing, transition steps, and eligibility vary by carrier, location, enrollment period, and individual circumstances, and are subject to change without notice.

Nothing on the Site constitutes a binding offer of insurance coverage. Any enrollment, plan changes, or coverage decisions are subject to the terms of the applicable carrier and plan documents.

2. Eligibility

You must be at least 18 years old to use the Site or engage our services. By using the Site, you represent that you meet this requirement.

3. SMS/Text Messaging Terms

If you opt in to receive SMS/text messages from Synergy Health Plans:

• Consent to receive messages is not required as a condition of any purchase.
• Message and data rates may apply. Message frequency varies.

• We are not liable for delayed or undelivered messages due to carrier network issues.
• Supported carriers may change without notice, and carriers are not liable for delayed or undelivered messages.

4. Communications Consent

By providing your phone number, email address, or other contact information to Synergy Health Plans, you consent to be contacted by phone, email, and/or SMS/text message regarding your coverage transition and related account matters, in accordance with the Telephone Consumer Protection Act (TCPA) and other applicable law. You may revoke this consent at any time as described in Section 3 above or by contacting us directly.

5. No Professional or Medical Advice

Information provided through the Site is for general informational purposes only and does not constitute insurance, legal, financial, or medical advice.
